My Pella Health provides you with online access to your medical record.

This patient portal technology from Pella Regional Health Center is designed to help you better manage your health and your family’s health.
Patient self-scheduling is now available!
If you are enrolled in MyPellaHealth Patient Portal you can now schedule your next appointment with your Internal Medicine or Primary Care provider through the portal. If you need an appointment for one of these visits, try self-scheduling:
- Well Child visit
- Family Practice Office Visit
- Internal Medicine Office Visit
- Family Practice Complete Physical
- Internal Medicine Complete Physical
Login to MyPellaHealth Patient Portal and click on 'Appointments' from the main screen. Choose the appointment type and click next to see the available providers and locations to choose from. Patients will have the ability to schedule same day appointments if available and can schedule appointments as far out as one year. Patients must be enrolled in MyPellaHealth Patient Portal and have been seen by the provider in a previous visit to self-schedule with that provider.
Access to My Pella Health Patient Portal from Apple iPhone Health App
Patients using an iPhone iOS 11.3 or later and who have a Pella Regional Patient Portal account can easily view their Pella Regional Health Center health records using the Health app on their iPhone. To access your health records on your iPhone, go to the Health Records section of the Health app and search for Pella Regional Health Center. Login using your Pella Regional Patient Portal account username and password. Information including immunizations, lab results, medications, vitals and conditions and procedures can be viewed.
MyPellaHealth Patient Portal Enrollment Request
Please fill out the form below to submit your request for enrollment in the Pella Regional Health Center MyPellaHealth Patient Portal. This form is only for self-enrollment and not as a request to access another person's health care information (for this, you will need to navigate to Proxy Access Request (pellahealth.org) and return the form and any documents to Health Information Management in person or you can contact the portal support line at 641-628-6792 or e-mail email@example.com). After you have filled out the form below for your own self-enrollment, you will be sent an e-mail with a logon link within two business days. If you do not receive an e-mail from firstname.lastname@example.org, within two business days, please email email@example.com. If you would rather fill out a paper form instead, click Enrollment on the right. The enrollment request form isn’t to be used for requesting a new patient portal password.
