Pella Regional Medical Clinics Achieve Level 2 Patient-Centered Medical Home Recognition

PELLA, Iowa— Pella Regional’s Medical Clinics in Pella, Bussey, Knoxville, Monroe, Ottumwa and Sully have received Level 2 Patient-Centered Medical Home Recognition for using evidence-based, patient-centered processes that focus on highly coordinated care and long‐term, participative relationships. This recognition, which is valid for 3 years, comes from the National Committee for Quality Assurance (NCQA).

“We know that our medical home provides patients with better quality, patient experience, continuity of care, prevention and disease management,” said Lezlie Morris, chief of clinic services at Pella Regional Health Center. “This recognition encourages and enables providers and their care teams to manage their patients’ care continually, rather than simply providing care once patients are sick or injured.”

The NCQA Patient-Centered Medical Home is a model of primary care that combines teamwork and information technology to improve care, improve patients’ experience of care and reduce costs. Medical homes foster ongoing partnerships between patients and their personal clinicians, instead of approaching care as the sum of episodic office visits. Each patient’s care is overseen by clinician-led care teams that coordinate treatment across the health care system. Research shows that medical homes can lead to higher quality and lower costs, and can improve patient and provider reported experiences of care.

As a patient centered medical home, Pella Regional’s responsibilities include:

  • Providing the highest quality care possible using evidence-based medicine and providing patients with support and self-management tools to reach personal health goals.
  • Addressing all aspects of a patient’s health, including preventative services, acute/sick care, management of chronic conditions, and behavioral health needs.
  • Coordinating patient care with any specialists or outside services necessary to best meet needs.
  • Providing resources 24 hours a day, 7 days a week. Beyond Pella Regional’s normal medical clinic hours which extend beyond 5 pm we offer a 24/7 Nurseline and the My Pella Health Patient Portal to ensure health care needs are met.
