Our team of professionals includes Registered Nurses, Licensed Practical Nurses, Home Care Aides, and Rehabilitative Therapists. We believe that home is the best place to recuperate after a medical crisis. We want to help our patients maintain an active and meaningful life.
Services
We offer the following services:
Initial Evaluation Without Obligation
A registered nurse will first visit to assess your health care needs and determine how your service will be funded.
Skilled Nursing Services
A registered nurse will regularly re-evaluate your plan of care, monitor your progress, communicate with your physician, and administer any services requiring specialized nursing skills.
Home Health Aide Services
Aides assist with personal care and perform services needed in your treatment plan.
Rehabilitation Services
Physical therapy, occupational therapy, and speech therapy are available as needed.
Special Equipment
We can help you obtain supplies and equipment essential for your home health care.
Home IV Therapy & Home Phototherapy
We are able to provide intravenous therapy services and phototherapy for infants.
Educational Services
We provide you with individual training, such as diabetic teaching, to help you care for your health. We may also provide community education.
Medical Social Worker
An MSW will assist you with emotional and social aspects of home health care.
Other Home Health Care Services
The Home Health Care Agency also offers a homemaker service called Helping Hands and Hearts. Trained homemakers provide light housekeeping, errand running, companion care and stand-by bathing assistance.
Respite Service is available to relieve your normal caregiver (spouse, friend or family member) of his or her duties for up to six hours.
