For your convenience, we offer different options to request medical records.
You may click here to download a PDF of the Release of Information Form. This option is for patients who are unable to come to Pella Regional to sign the release form. After completing the form, you will need to fax it back to the fax number listed in the upper right hand corner of the form.
You may also call the Pella Regional Health Information Management Department to request medical records at 641.621.6662.
The Health Information Department is located on the lower level of the hospital close to Pella Specialty Clinic. For your privacy, medical records cannot be faxed directly to patients.
