Health Management Program at Pella Regional’s
Family of Medical Clinics

PELLA, Iowa –Pella Regional Health Center’s family of medical clinics in Pella, Bussey, Knoxville, Monroe, New Sharon, Ottumwa and Sully are now using a new automated patient health management program designed to deliver better coordinated, more personalized care. When it’s time for patients to attend a scheduled appointment, schedule a preventative care or follow-up appointment, or manage chronic conditions, they will receive a friendly communication from Pella Regional in the form of an email or a phone call.

“Our goal is to ensure that you receive the care you need, when you need it,” said Lezlie Morris, chief of clinic services at Pella Regional Health Center. “We are excited to implement this new service throughout our medical clinics and believe it will enhance the care we provide.”

The program reaches out to patients so that clinicians can better monitor their health care needs. This includes scheduling patients for preventative care services, such as annual wellness exams and meeting with patients who are overdue for checkups related to chronic conditions such as diabetes and hypertension. Employing nationally validated, evidence-based protocols, the technology triggers automated communication to patients when they are due for these preventative or chronic care services.
